Fall 2023 Feedback Form

As we approach the end of the semester we would like to give each of you the opportunity to share feedback with the board members. Regardless if you attended just 1 meeting or 10+ meetings, we would like to hear from you. 

All the questions on the feedback form are optional, but please fill out as many as you can. Your feedback is critical in helping board members better meet the needs of club members.
